Wishing Tree Cards

Ok ladies.. someone was married within the last two weeks and posted their pictures online. I saw them about two Monday's ago. In the photos the bride had a wishing tree and wrote a really cute poem of instructions for people.  I need to know how to make the wishing cards for the tree. I have seen one option at the link here:

But if anyone else can help me with ideas or ways to make these cards that would be great! Also if anyone could tell me  who that bride was or you are that bride with the wishing tree PLEASE tell me how you did it and where the poem came from. my FI says he can wrote one but I have never seen him write any type of peotry before... but may he can..

    haha I just saw that picture again today. Two Knotties used the same photographer and I found it! Yes, that is one. BUT I thought I saw another one... but that one is good enough! Thanks!!
