Addressing the Invites: Question about titles

How do I address the invites for these two situations. 

Couple number 1 is Mr. Cruz and Captain Cruz. Do I send the invite to Mr and Mrs Cruz or Mr and Captain Cruz? Or something else? 

Couple number 2 is Doctor/Captain James and Doctor/Captain James. Do I address the invite to Mr and Mrs James, Doctor and Doctor James, Captain and Captain James? Or something else?

    Thank you. A follow up question. I was always taught that you put the husband's name first when addressing mail. Is this not true?

    Not if you are using both of your names.  If you are using your titles, you are Mr. and Mrs. John Doe.  If you are using your names, you are Ms. Jane Doe and Mr. John Doe.  Mrs. Jane Doe is incorrect, but is sometimes used for divorced ladies.
    Yes, the rules have changed a bit.
