NWR - Job Application Question...

So I'm filling out a job application to work for the State of Oregon...and they want to know every job I've held in the last 10 years...my very first employer was a family owned place and has since then gone out of business. On the application it asks for the address and phone number of the business and I have neither. I can try to find the address and phone number but should I bother, or just write in that space, "Business has since closed" or what? TIA

i would write something along the lines of "no longer in business"....then city, state, zip. so at least they have an idea on where it WAS....

Ditto Jennlin. Give city, state and zip if you can find it and put note in phone number space.

That's what I'd do. I'm sure MOST people don't have their manager's number from some random job 9 years ago. Probably happens all the time.

I agree with PP... I just got done looking through a TON of resumes. That is what I would expect to see.
