Before we go online I want to have all of the information lined up in case I need any documents. Do you need stuff off your birth certificate like information about parents, where you were born, etc?

You can just look at the online application without filling anything out, but yes, info from your birth certificate, any previous marriage end dates and parents full names and State that your parents were born in - or you can type UNKNOWN in the fields if you want.

I know this is an old thread, but when you sign the license do you sign with your maiden name or what will be your married name?

your maiden name...cuz you are still that person until you change your social security info.

The website says they may ask for proof of age? I didn't plan on bringing my birth certificate. Did this happen to anyone, where they asked?

Your driver license should be sufficient proof of age and identity.
