mad lib cards for the reception

Hi there i was wondering if there is anywhere on line that i can purchase either the actual cards or a template to have mad libs at my wedding reception? I'd think it would be alot of fun to read what my guest write. but i'm having a hard time finding someone that actually sells them?

My MOH had some made at vistaprint! The idea is that the guests fill them in, then if they are comfortable they can go to the mic and read them for laughs! We are doing those instaed of clinking glasses for a kiss!
