  Seriously, have you checked out toy stores? You might be able to find something appropriate (maybe little action figures) that could be used as a topper, even though technically isn't one. If you can find something, attach them to whatever you think would be a good base and you're all set. We created our own topper using Benders flexible characters. It worked well for us. (I think there's a pic in my bio, but if not I can post one if you want to see.)
  
  Another idea... instead of actually using figures or dolls, how about a small nurse's hat and fireman's helmet on top next to each other? Maybe even incorporate a veil onto the nurse's hat, and bowtie peeking out from the helmet?

  I like the hat idea from PP. Also kissing sillouettes with the different hats come to mind  that could probably be handled by some  laser cutting acrylic place - I had my last name done by the following company for my wedding topper (not to say they can do human sillouettes but its a start!).

  Have you asked your cake person to make this for you from gumpaste or fondant?  Many cake designers do this now.
