Getting in shape on a busy schedule

I'm currently halfway through a medical residency (finish in July 2012!) and will be getting married September 15, 2012. I really would like to lose 30 lbs before the big day but it's so hard to find time to work out with a workday that lasts anywhere from 12 to 15-16 hours.  Any suggestions on some fast, at-home things to do that can get results?  I do go to the gym at least on the weekends.
