Getting in Shape

My diet-What am I doing wrong?

Here is pretty much my daily diet-It doesn't look bad to me but im not losing anything and my pants keep getting tighter

Breakfast:
7/11 Coffee 12oz
Rocket Chocolate (70 calorie Caffiene)
Banana

Mid Morning:
Activia
Baby Food

Lunch followed by a fast paced 20 minute walk:

Vegetarian Soup & Buttered Roll
OR
Egg or ham & turkey Sandwich
OR
A Ceasar salad (with no meat or crutons), minimal dressing

Dinner:
Activia
