    Looks good to me.  FI would ask "What's the M and line for?"  (Thats what he said to me when I asked him to proof it before printing. LOL)

    That is how mine were - yes you are going or no you are not and if you are going, what do you want to eat. Pretty self explanatory.

    I think it makes perfect sense! Now, it's my understanding that people will still find a way to mess it up, but that's not your fault (you made it as self-explanatory as possible).

    Ugh we had a bunch of text responses, it was obnoxious. 

    Yes, your RSVP card looks great and is easy to understand.  That being said, people will still mess it up.  Ours were similiar except I replaced 'M' with 'Name' (I didn't know what the 'M' meant...) and we included one RSVP card per invited guest.  We had a bunch of people return one card for two people.

    My favorite was FI's friend who returned the only card we sent him with his brand new girlfriend's name on it and a 50$ check as a donation for her dinner.  I FLIPPED out when I saw it but now think its funny...
