Addressing invites to priest, monsignor, deacon?

Could someone please help with the proper titles for a Catholic priest, monsignor, and deacon on wedding invitations?  I've found several conflicting answers online, and no one in my family seems to know.

    I believe we did:

    The Reverend John Smith for our priest...but we might have done Father John Smith.  i honestly cant remember.  sorry, that isnt much help!

    You may want to ask your church office for the proper way for a Monsignor. However, if my memory serves me correctly, I think it is The Most Reverend John Jones.

    For our priest, it was The Reverend FirstName LastName on the outer envelope, and on the inner envelope, we put Father LastName.

    Transitional deacon: Reverend Mr. or Deacon
    Permanent deacon: Deacon

    Diocesan priest: The Reverend
    Priest who are members of religious orders: The Reverend Father
    Priests who are abbots of monasteries: The Right Reverend

    Monsignor: The Very Reverend or The Reverend Monsignor (this one isn't as clear as the others because monsignor is an honorary title and not a part of the Sacrament of Holy Orders so the titles change culture to culture)

    Bishop: The Very Reverend or The Most Reverend

    Keep in mind that this is just for the Latin Rite Catholic Church.  It will change for those who are in the Eastern Catholic Church.

    Hi - Sorry to hijack the thread, but I have a follow up question. Our priest is a Jesuit, should I put "S.J." after his name on the invitation and in the wedding program? I noticed that "S.J." is included in everything he signs.
