Passport renewal

Hello ladies! I haven't been here in months! And now I need some input...

I'm not eligible to renew my passport with my new name for free because I've had it for longer then a year. So, I'm filling out the renewal by mail form and sending in all the required stuff to get it changed. I'm stuck on one question...

On the application it says "name change information." Complete if name is different than last passport book or passport card. So I select "changed by marriage." It then says "place of name change (city/state)" and "date (mm/dd/yyy)." I also have to include a certified copy of the marriage certificate. Here's my question - do they want the date of the wedding or the date I legally changed my name? I would lean towards the date it was actually changed by the soc sec office but I just wanted to be sure.

  • I would think it would be the wedding date, but I'm not sure.

    Also, I don't know if this is normal, but when I applied for my passport (I didn't have one before getting married), I had to provide 3 different documents in addition to the marriage license to prove that I use my married name instead of my maiden name. It was weird. I had to mail a copy of my health insurance card, car insurance card, and I think my driver's license counted as the other form.

  • I should do this.. urgh. The things you forget to change. I was at the dentist a few weeks ago, and saw my chart and it had my madien name.. luckily it was easy for them to change it. Still need to call my doctor about that too..

    Oh I know, I keep coming across things too. And sometimes I get things in the mail with my maiden name and I've already changed my mail with that company (like my bank) so I'm not sure why that's happening.

    Okay great thanks! I tried googling and couldn't find the answer. Did you staple your new pic to the paper application? I wasn't sure about that either...I got it done at kinkos and they put it in a nice little insert for me, with two pics. I'm not sure if I should be sending both or not. I guess it doesn't hurt, I'm not going to use it for anything else. Looks like a bad mug shot. :)

  • I'm no help - but I need to get going on changing my name. DH is starting to get annoyed with me lol. Not looking foward to all that!

  • I believe your supposed to put the wedding date, because that is the actually legal day you changed your name; since the marriage certificate is a legal doctument.

    I need to apply for a new passport; mine expired last year and I didn't feel like getting a new one to jjust have to do all the paper work to get a new one (free or not).  So eventually, I'll be getting a new one.

  • I did staple the photo to the document. I think, technically, no you did not change your name on your wedding day. For instance... what if you don't plan on changing your name? The wedding date is just the allowable instance that you can change your name, if you choose to.

    Pharmacy was something I completely forgot about. I had to get an antibiotic and when I came to pick it up they said they couldn't find me in the system. I said "I just got new insurance....... OH and a new name."
