Invitations and menu choices

How do I phrase my response card so that I can determine who has ordered which menu selection?  I need more than a general head count of steak vs. chicken so I can print each persons's specific selections on the table cards for the waiters to see.

You could a spot on the response card where they initial next to the entree.

"M_________________

____will be attending
____will not be attending

Please initial next to your entree of choice:

____Steak          _____Chicken"

what is the deal with the "M"?
and do I also need to list the Vegetarian option and kids meal choices?

I just put the total number of each choice in pencil on the back on the table numbers (i.e. 4 chicken, 3 fish). The server would just look at the back of the card and bring out the appropriate number of dishes.

M___________ = a space for them to write in their names. Mr. & Mrs. John Smith

Ms. Jane Doe.

You can write vegetarian and kids or write "Please indicate any dietary restrictions or allergies" at which point someone will probably put "vegetarian" or "only eats chicken fingers."

be careful with the initials, I ran into the problem of families with more than one person with the same initials.

This is exactly what I did and it worked great. Nobody was confused, and nobody added in extras. I guess I will find out on Saturday if people are going to show up un-announced, but I'm not expecting anyone to.
