Reception dinner - Menu Selection

Hello!
I am having a very small wedding with about 40 guests (many of which are children). We are holding the ceremony & reception at my sister's home. The wedding and reception are both formal, just a very small guest list. My aunt and nieces are going to be serving the meals on the plates to everyone to keep the traffic down because we just don't have that big of a room. We don't want to serve menu items that someone does not like, and it would seem to go much more smoothly if they don't have to wait and ask each person if they want a certain item, especially the children. Something that was suggested to me was that I place a menu selection card along with the RSVP card when I send out the invitations. The menu selection card would show a place to write each guest's name then choose to have the full menu or they could select the items they do want. It seems like such a helpful idea, but I don't want to seem tacky if I included a card like this. What would etiquette say about something like this? I can't find anything specific on the Internet.

Thanks so much!!
Rhonda
