NWR: A missed Thank You (how do I fix this?)

Firstly, I know I messed up and I committed a terrible faux pas, and honestly feel terrible about it but have been unsure of how exactly to go about fixing it.

A few months ago my Father introduced me to an industry contact who was kind enough to meet with me and show me some of his business interests in my field and even asked for my resume.  This took place while I was out of state on vacation.

My intention had been to find a nice card and a local gift from where I'm living now to thank him for his time.

However, I am a schmuck and got wrapped up in my own world and a whirwind of BS at work, and long story short, I never got my card or gift, let alone sent them off.

Is it too late to send him something?  How do I apologize for the delay in expressing my gratitude without sounding like an A-hole (if there even is a way)?
Is there something I could/should say in my correspondence?

 I am so embarassed that I didn't get this done as soon as I got back home, but that's in the past and I can't change it now, and I'd really like to repair my mistake. 

You ladies have been very helpful in the past and I appreciate your suggestions.
