HR question - quit or fired?

I work for a pretty small company, so we don't have an HR department. I AM the HR department, in addition to a variety of other things. I had an employee request 2 weeks off during a busy time of year for us (retail company). I said ok, even though it was bad timing.
Then like a week before it's time for her vacation she said "Oh yeah I'm actually going to be gone for a month." Not ok. I told her if she were to take the whole month off I was going to have to find someone to replace her. She said that was fine, no big deal, thanks it's been nice working here.
Now I'm filling out unemployment paperwork, and I don't know how to explain the situation. Yeah she sort of got fired, but she had the choice to stay and chose her vacation. Thoughts?
