XP: RSVP wording

I need some input on our RSVP wording, please.  We have to find out who wants each entree so I can indicate it on their escort card at the reception venue.  I'm not 100% satisfied with what I have so far, but here it is.  Suggestions appreciated!

Please write each guest’s name next to the entrée he/she would like.
Dinner entrée choices:
Parmesan Crusted Scrod_____________________________
10 oz. Boston Strip Steak ____________________________

Children’s entrée choices:
Chicken Strips with Fries and Applesauce _____________
Bowl of Pasta with Applesauce _______________________

that seems like a lot of words. I just sent in a RSVP card instead of words for the food, they had pictures of a fish or a steak etc. and next to it a place to write the #. Ive never seen it that the venue had to know which guest had what. Ive only ever seen Table 2 has 3 fish and 4 beef.

If you don't have tons of kids on your list, you can just call after their parents' RSVP comes in and ask which meal they want.

There are only about ten kids on our guest list who would want a children's meal, and some of them live in the same household.  We don't have a meal option on our cards, but I'll call to see who wants chicken fingers and mac & cheese.

And we're sticking a note in the invites with kids about indicating how many they need, since it was wordy to begin with and there aren't that many kids.
