Addressing an invite

I feel like I should know thte answer to this but I don't!  

If the couple is married but the wife kept her last name do I list them Mr. Rich his last name and Mrs. Barb her last name?  Or is it Mr and Mrs Rich his last name and Barb her last name?  

Then on the inside envelope same thing?  I don't know why this one is stumping me.

Help please!

Outer: Mr. John Doe and Ms. Jane Smith Inner: Mr. Doe and Ms. Smith If it won't fit you can put it on two lines as long as you include the 'and'. The and implies they're married. Some women who kept their last name may be offended by being addresses as Mrs. Husband's Name.

The woman is always listed first.  Yes, use Ms if she uses her own last name.

Ms. Jane Doe and Mr. John Smith
