RSVP card wording?

Question for you ladies... 

I'm planning to make a seating placecard arrangement with a placecard for each guest.  For this, I'll need to know the names of each of my guests.  Of course I know the majority of them, but I'm inviting single guests with a +1.  For example, John Smith and guest will be invited, but for the placecard, I'll need to know who is attending with him. 

Is there any tactful way to word the rsvp card such that the names of those attending are listed?

    I think they'll write it in (I always did when I was invited with an "and Guest").  So far, all but one of our guests invited with an "and Guest" wrote the name of their guest in on the RSVP card.  We'll call the one who just put "and guest" on his RSVP card to get the name (although I suspect he wrote it that way because he doesn't know which lady will be the lady of the hour for our wedding...but that's another story).
