  I did Jane and John Smith or Mrs. Jane and Mr. John Smith- i don't recall which.
  
  Jane Smith and John Smith is awkward.

  It should be Jane and John Smith.  Ladies first and you don't separate the man from his last name.

  We did separate cards for each guest (even though all guests were seated at the same table as their spouse/significant other/date).

  
  

  This way, your guests can place their escort card in front of their seat while they mingle, so others at the same table know who is sitting where (and don't take a seat that is already "taken").
