I "third" the edible pearl answer.

We have a cake & candy supply store near us and I bought a bottle and supplied it to my cake baker to use since the skirt on my dress had pearls sewn on it.

Pearls would have a sphere shape all around. If you look toward the sides of the cake you'll see the dots shapes are more similar to those of those old candy you eat of the parchment rolls. Must likely made of same thing as peals, but if your trying a DIY I believe circuit cut outs or icing every dot by hand may achieve the look more.
