Outer & Inner Envelope Addressing Question

I've been searching online for about half an hour now and every website says something different about how to address/label the outer and inner envelopes.  I think part of the problem is that one way is traditional while the others are more modern and then some of the modern versions are super casual.  I don't need/want them to be super formal, but I also don't want them to be too casual either. 

How did you address/label yours if you did both an inner and outer envelope?  I'm especially interested in situations involving kids who are invited and when someone is invited with a guest. 

Do you think these are too casual?

OUTER ENVELOPES:
**Just a couple**
Mr. and Mrs. John Smith
Address Line 1
Address Line 2

**A couple with kids**
Mr. and Mrs. John Smith
Johnny and Susie
Address Line 1
Address Line 2

INNER ENVELOPES:
**Just a couple**
John and Jane Smith

**A couple with kids**
John and Jane
Johnny and Susie

**A person with a guest**
John Smith and Guest

We did just that...almost.  

On the outer for people with kids, we either just put the parents' names or put "The Smith Family".  Then the kids' names were on the inner.

We didn't put any last names on the inner unless it was an unmarried couple with different last names where we were basically indicating by the inner envelope who their "guest" is.

Oh, and for outers for a friend with a guest, we just put the friend's name and then on the inner, put both of their names if we knew the guest's name (which we did for all but like 2), and then put "John Smith + Guest" if we didn't.

I am only doing outter envelopes, but for those with children I am writing "The Smith Family".

Ditto with "The Smith Family". We didn't have any inner envelope so we addressed single guests as "Mr. John Smith and Guest".

I did it like you did except I also put the Smith Family.  For my aunt, uncle and daughter I may have put Mr. and Mrs Dave Smith on the outer and then Dave, Jane and Jessie Smith on the Inner.

Ditto-if I invited an entire family, I wrote "The Smith Family" on the outer envelope.

I THINK the correct way, is no kids names on the outer envelope if they are under 18? Their names would only go on the inners...and over 18 should get their own invite...but I could be wrong.
