Escort/Place Card wording

How did you do your wording for these? Did you give each guest their own, or each couple?
I'm not officially working on them just yet I'm going to do it after I get my RSVP's back in May, but I just started thinking about it and typing a few things up for the people I know with be there.

Example:
(For married couples)

Mr. and Mrs. John Smith

Mr. John Smith
Mrs. Jane Smith

Mr. and Mrs. John and Jane Smith

I'm confused!
Thanks :)

Is it appropriate for unmarried couples to be listed just by name?

    Example:

    John Smith and Jane Brown

    or should it be:

Mr. John Smith and Ms. Jane Brown
    Miss? Jane Brown?

    Thanks for the help!

For married couples, we did Mr. and Mrs. John Smith

For unmarried couples, you use their names on two lines:
    Mr. John Smith
    Ms. Jane Doe

You do not connect the names with an "and" because that signifies that they are married, teh same as on your invitiations

Our place cards as well as inner envelopes were very casual

    For married couples it was:
    John and Sarah Smith

For unmarried couples (not many) it was
    John Smith and Sarah Jones

    And for singles it was:
    John Smith
