  I made a 20-page photo guest book with 10 pages blank for people to sign. We will have about 170 people at our wedding, so I figured this would be plenty. I put lines for people to sign their names on/write notes on. I also added in quotes on the same pages that I put pictures.

  I haven't made mine yet, but I'm not planning on any blank pages.  I'm thinking of having the phots be against a light background and having guests sign/leave messages around them.

  I didn't leave any pages blank. But I did set up the pages so that there was blank space on each page. Like this:

  I put a watermark of one of our pictures on the side where the guests will sign. This way, if that page ends up not having any signatures on it, it won't be completely blank. Plus, I didn't want to put lines because I figured we'd be more likely to get actual messages/notes if it didn't look like the typical guestbook format.

  I'm using shutterfly because I have a $20 gift card....  It's been pretty easy to work with- once you get started!

  Mixbook has a template for "guest book" that includes some lined pages as well as room for pictures. Just another option...

  I think our book is about 20 pages and about 5 of them have lines for guests to sign. There will be enough lines for all of the guests to sign their own name.
