help with response cards with menu selections

in May 2013 Weddings
so i am working on the wording of my response cards and nees some input. So far it reads: 

The favour of reply is requested by ____(date)
M________
___ will attend
___ not able to attend

indicate number of meal selections
__ chicken
__ steak
__ vegetarian

should i leave it as it or change it to say: initial meal selection for each guest

idk if this will help in that i will know who ordered what at each table or which of the two sound better. Suggestions?

    Ours will have an area for each person invited to write their name if they are attending, and next to it is a list of each menu option i.e. Chicken, beef, vegetarian which they will then circle their choice. There is also a box to check if the entire party is unable to attend.
