The kits at Michaels were by Brides.com in the wedding section.  They don't sell them online, but they do have the info.  The kits come in sets of 40, and they have the instructions on how to download the templates.  You can customize the font and font color, and we added our monogram to the first leaf.  There are usually several styles to choose from.

    Thank you soooo much for this rec!  I got the tri-folds from Michael's and printed our OOT bag booklet on them and they turned out SO great!  It was less than $30 to make all of them - and they def turned out better than if I had them printed anywhere else!
