What do I need when I go to SS office to change name?

When I go to the Social Security office to change my name, other then the completed application and my marriage certificate and drivers license, what else do I need? Do I need my SS card as well and password? After I go, can I immediately go to the  DMV or do you have to wait a few days? I couldnt figure it out on the website thanks so much!

I think what you said is all you need. Once you complete the application they mail you a new card about a week later, however you do not need your new card to go to DMV as long as you have the 6 points of ID without it.

This was really helpful for me (from a previous mod):

Thank you! With the 6 points of ID, since none are in my new name yet, is that ok as long as you have the marriage certificate? Thanks so much for your help!

I changed my DL before my SS card.

You just need the six points of ID (maiden name ID) plus your marriage certificate to change your driver's license. I didn't have an SS card (lost it), so I used my old license, birth certificate, bank mail (since I didn't have proof of current address). None need to be in your new name, just have your marriage certificate.

For the SS card, you need a state-issued photo ID (I used my passport, you can also use your DL even if it's not changed yet) and your marriage certificate. You need the form from the SS website regarding getting a new card. I did not need my current card.

Your name is legally changed whenever you change the first one of those two things.

There is an online document selector for the 6 point system for the DMV, so you can see exactly what you need before you go there.

I did the drivers license first.  They only asked to see marriage license and maiden name ID.  They didn't want to see anything else that I brought as backup points.

For the dmv I went to in south jersey you NEEDED your birth certificate for your six points, passport wasn't even good enough.
