Back from Mexico! ANd need to change my name!!

Hi Ladies!!  I'm back from the Riviera Maya!  It was PARADISE and we were sad to go, but I was also anxious to get home to our 2 year old son and was so happy for that.  :)   

So today I need to change my name and I can't find the form I got when we went and applied for our marriage certificate.  Can I just go to the court house or the DMV and they have everything I need?  Oh and we did get the notarized copy of our marriage certificate in the mail already so do I need anything else?

    Welcome back!

    All you should need to bring to the DMV or wherever is the notarized copy of your marriage certificate. That was all I needed to get a new DL, bank card, etc...

    I needed a copy of my notarized marriage certificate (do you have yours back yet even? Mine toook like 3 weeks, I think.) and my current Driver's License for the new SS card.  

    ETA: duh. I just reread your post.  You don't need the actual form from when you applied for the marriage certificate... you just need the actual certificate.

    I'm glad you guys had a great time! I bet it was gorgeous down there, and then to come back to the snow :( At least the weather is supposed to be looking up from here anyway... good luck on the DL change!

    Thanks Ladies!!  Yes we left last week and drove to the airport in the snow.  It was a nice contrast though arriving Mexico in 80° weather.  :)

    Now though, I couldn't be more ready for SPRING after a taste of that!!
