Does this wording make sense? I'm including a small buisness card sized paper in my STD's to let my guests know about our hotel block...

    Is there a minimum number of nights guests must book? Otherwise it is exactly what I did except we had two hotels with different price points.

    Overall, it looks good.  The following words don't need to be capitalized: accommodations, wedding, reception.  I'm not sure if you just formatted it like this beause you aren't putting in the actual phone number and address, but you don't have to say "on" or "in" - just list the address and phone number.  Also, in the last line, I would use "please" instead of "must".

    Instead of "please" or "must" book by, I would phrase it as "rooms will be released by DATE" or "the rate will be honored until DATE," etc. Someone not spending a lot of time thinking about this or who isn't used to room blocks might not understand it's a deadline rather than your planning preference.
