What to include on the Photography Contract?

The photographer that I would like to book has a contract, but I want to be sure that I am as covered as she is if by chance something goes wrong. I know she has taken care of her concerns in the contract, but I will feel comfortable knowing that any concerns on my side are addressed as well. The only thing I have noticed that I believe should be included on the contract is the exact price I am paying her for her services and the exact times and days I will be needing her. It seems to be a good contract, but I want to know what ABSOLUTELY must be included in a contract. I don't want to be an overbearing bride, it's just that this is my big spend for the wedding and something extremely important to me. I've heard too many horror stories. Any advice or stories to learn from?

-A turn around time for receiving items post wedding (edited photos, cd, albums, prints, etc)
-What happens if they are ill or injured and cant show up on the day of.
    -Cancelation policy and fees.
-If you are getting a disc of images: Are they high resolution and that you get reprinting rights.

Thats all i can think of off the top of my head.

    Be sure to include:

    *  The price for the services, as well as the details of your package.
    *  The amount of money you put down, as well as any payments that are to be made before the wedding.
    *  Location of the wedding, reception, and any other locations you would like pictures at.
    *  Date at time of the wedding as well as the time you expect the photographer to begin taking photos.
    *  Any and all special instructions or package information that you have agreed on
    *  Cancellation terms (for both you and the photographer), in case someone can't make it to the event, or in case you decide you are not happy with the photographer's work (like you get your engagement photos and they just don't work for you) and you want to get out of the contract.
    *  How long it will take to get your proofs, album, prints, etc.
    *  Copyright release information for digital negatives.

Remember that the contract is there to legally protect both you and photographer, so make sure everything is correct!

Is it usual and customary to pay for the photographer in full before the wedding?
