For your RSVP: I would just say "The favour of your reply is requested by...." On or before sounds a little wordy.
For your Reception Card: I don't think you need to include "located at:"-- just have the address listed Also, you can just say "four o'clock in the afternoon" you don't need to give them a break down of the entire schedule for the evening and the meal times.
Also, I would just discretely include the web address at the bottom, you don't need to explain it's purpose.

I get a sense that your wedding is very formal. Since mine is relatively laid back, my advice might not be of use, but I hope it helps.
RSVP:
You could change your "Yes" option to read "______ Number of guest in attendance" You also might want to include some sort of indication for guests to tell you how many of each entree they would like, if that is important to know. I could see people just placing a check mark there sayin "Yes we want options A and C for our family of 5". Responding a month before seems a bit early to me, especially if you send them out six weeks prior.
Reception:
"The reception of our union" seems oddly worded to me. Maybe "reception following our union" would sound a little better. Formatting wise, I think it would look better if the restaurant was on it's own line or two. As for the website, I would either just have it alone at the bottom or say "For more information, please refer to our website XYZ"

Ya'll are so helpful! I totally agree with the fact that I need to know how many people will select the entrees. What is the best wording to inform them of that?
"Please indicate the number guests that would like the following entrees"
Or if you need to know who wants what dish "Please place your initials next to the entree you would prefer" or "Please write your name next to your preferred entree"
We're doing a buffet, but that's how I would word it for a plated. HIH

Yeah... do you need to know exactly who is eating what or just the numbers? If you need to know who is eating what for a plated meal, you might want to ask them to initial their preferences. No matter how you word your RSVP I guarantee you will have to make some phone calls to clear things up, all of us probably will!
