help addressing escort cards

So I have managed to confuse myself on this one...am I supposed to put titles (Mr. John Doe) or is it OK to just put John Doe? If I am supposed to put titles, do I put Dr, The Honorable, COL, etc??Help! I really want to just put first and last name but is that tackY??

    On everything I read, the escort cards are supposed to be formal...but I didn't do them formal (I didn't have formal cards, though) unless it was Dr.  I just put first and last name.. nobody will care. :-)
