Name changing problem

I had never thought about this, until my mom asked what I was going to do.  My cousin just got married, when she changed her name she dropped the middle and put her madian name there then took her husbands last name.  I had never even thought about doing this but after talking to some of the RNs, PCAs and MDs at work I found out this is very common.  My question is What did you do or what do you plan to do?  I want to take my FIs last name, I just don't know what to do inthe middle.  I know there are a few nurses on the board. Do you think we will have a hard time changing our names with the boards, KY and OH?

I plan on tacking his last name on the end of mine; I'll have two middle names.  His mom has the same first and second name as me, so I can't have the same name as his mother!  That's just creepy, right? :)

Changing my last name to his, leaving my middle name as it is. The fewer changes the better.. even though my new initals will be KMA. :) Fwiw, I am not in the medical profession, however I do have to be licensed for my job. Once I have my new SS card, I will have to have my license updated. Another girl in my office was married 2 years ago, she did not have a problem at all with getting her name changed it just took some time. HTH

I am taking FI's last name and dropping my maiden name all together.  I can't wait to get rid of my maiden name and I'm very excited to take FI's last name.I have also known people in the medical profession that have changed their last names to their husband's last names and it wasn't any more difficult.  It just took time like everything else.

This is something I considered for a long time.  I am a bit of a feminist when it comes to my name.  I already have 2 middle names and I am not dropping one of them.  I am just tacking on my husband's last name.  I won't have a hyphenated name but I just don't feel I should have to drop any of my names if I don't want to.  My name had very specific value and meaning to me and getting married shouldn't mean I have to lose some of that. However I am going to the SS office this week to do it.  But the application only has space for three names so I am not really sure how all of this is going to work, I'm a bit nervous about it.

My current last name is left over from my last marriage.  Even though I will no longer share the last name of my children, I am pretty psyched to get rid of it.  I'll be taking FI's last name and never looking back!

I'm still contemplating replacing my middle name with my maiden name, only because I'm the last one of my last name in my family, and I like it more than my middle name. I'm an RN and from what my other nurse friends have told me it's not hard to change your name for your license, at least in OH. The BON has an info change form or something you fill out, available on their website. If you're still in school, it'll depend when you change your name with SS/DMV compared to when you apply to take the boards. If it's already done, just fill out your apps with your new name. Just make sure everything matches, though, because you have to do the FBI background check and I assume they match your SSN to your name on your app.

Most of my friends have dropped their middle name and used their maiden name as their middle. My middle name is also the middle name of my mother and grandmother and I plan to pass it on to a daughter if I have one, so I will be dropping it.

I'm in the medical profession and I am dropping my last name and taking his. I'm keeping my middle name. Personally, I think making a former last name your new middle name is weird (unless you didn't have a middle name to begin with). But, to each his own. It's not hard to change your name with the board. You send a copy of your marriage license in with a form and voila! name changed. I'll be practicing pharmacist with my new name right after my wedding. It's easy.

I went to the Social Security office and was able to make my maiden name my second middle name, taking my husband's last name.  When I went over to the DMV and they informed me I can't change my middle name in any fashion!?  So on my new license they gave me two last names seperated by a space.  I don't know what to do.  Did anyone have a similar experience?
