Marriage License Question

We are going to be getting our marriage license soon for our fall wedding.   For MN, on the license it says 'give the names the parties will have after marriage.'   I will be changing my name, but do not actually want to change it until a month or so after the wedding because for our HM my name is still my maiden one on our airline tickets.  

Does putting my name after marriage cause a name change or how does this work?   Should I put my old or new name there?

    Put down your new forever name.  Your travel docs, drivers license, credit cards, bank records etc don't get changed until you tell them to and can't happen until your marriage certificate arrives anyway.  If you don't  put it down correctly on that form, you'll have to go through a bigger hassle to have it legally changed.

    Ditto PP.  That will be your name, but all of those documents won't be changed until you do so yourself.  This won't cause problems with your HM.
