Changing last name on Passport

What'd the best way to go about doing this? My passport is the only thing I have left to change and I'm not exactly what to do. I printed the request form from the website, but what else do I need to do?

    you need to have a new photo taken, mail your old passport along with the new picture and the fee back to the department of state.  they will then mail your old passport back and your new one (seperately) within a few weeks or so.

    If you have had your passport for longer than a year, you basically need to reapply for a new one.  You need to send them your old passport, new photos, the fees, and the DS-82 form (which can be filled out online and printed), along with an original marriage license. I did it a few months ago, and with expidited service (needed it back before HM) I got it back within 4 working day (sent on a Saturday, in my mail on Thursday).     duh, the marriage license would help!i called to find out if i needed to send priority or not and the rep said no.  i think i got it 2-2.5 weeks later.

    it was pretty easy get 2new pic, print the form online, send the old passport and your marriage license if you got the old one less than a year ago then its free if its over a year you have to pay
