What did you do re Reply Card wording

I have a question on reply cards......

Example: I am inviting my cousin and her husband and two young adult children (17 and 23).
I am wondering how I would track their meal choices of.....

__ Beef
__ Shrimp
__ Chicken
__ Vegetarian

Should I suggest putting initials by the selections or just the number of selected choices?

    I did what ceh789 suggested and it worked out great.  I made my invites and it was labor-intensive but oh-so-worth it.  It also eliminated unwanted guests (we did no kids)
