    I know that on the application you only put their name, so I am assuming that is all that is included, but not positive....I haven't gotten mine yet.........

    On the application it just has their name (no religious affiliation) and on the actual marriage certificate it does not have any information about who married you (not their name or anything). The certificate just comes from the state of Maui and has a certification number on it and where you got married.

    On the actual certificate, nothing about the officient is included on the paper. My uncle had a TON of paperwork to fill out for marrying us though, as he's not from Hawai'i.
