  I don't have mine in my bio, but I can help you out by sending mine via PM. I have helped quite a few Catholic brides out already. I also have a lot of websites still bookmarked that have templates on them if you want those.

  My church had copies of ones from previous couples that we could look at and borrow. You might ask your church if they have some. I also used this one as a basic guideline:
