Name Change Checklist!

So since I am getting close to the big day I figured better figure out how to change my last name after the wedding. So I think I got all the paperwork in order. 

-Applied for marriage license
-Filled out Form SS-5 for IRS and S.S. card name change, got the address of the closest SS office
-Called USPS and requested a name change form to be mailed. 
-Passport name change form DS-82
-And of course renew my Drivers License

And made a list of all the places that need to be informed of my name change
-Bank (order checks/Debit Card)
-TV provider
-Power Co.
-Utilities (Propane & Garbage Co.)
-Employer
-Insurances
-Phone Co.
-Paypal

If I am missing anything please let me know!!!!

    I am a list girl... if I dont have my lists I feel like I am gonna miss something! Theres about 20 lists on my bio!lol

    I didnt even think about it till yesterday and then started freaking out because I hadnt thought about it. I didnt realize there was so many things I needed to do to change my name!!!

    That's exactly why I am not changing my name.

    I guess I am just moden.  I've had my name for 39 years.  People identify me by that name.  I don't need to take my husband's name to be his wife.  That's just my thoughts.  Plus the complication doesn't really outweigh the benefits.  FI is indifferent.  It doesn't affect him if I don't change my name.

    A couple other ideas to get change your last name with...
    mortgage (or lease if you're renting)
    store cards (like the grocery discount cards)
    club memberships
    credit cards

    I lucked out and have had an incredibly easy time changing my name on everything - I wish you the same luck!

    This is a good list!! I may print it too.
    I'm bad, got married in November, still haven't done any of this!!  It still feels weird to me to call myself by a different last name. I am changing it, but I can't get used to it yet, maybe that's why I'm waiting? lol

    Any investment companies, including any 401(k) accounts you have with your employer.
    I sign up for all kinds of catalogs and such.  Those will all need to be changed too.

    Also ones I didn't think about until later...

    vehicle registration,
    AAA,
    alumni directories,
    gym memberships,
    costco,
    dining reward stuff

    FYI....typically anything related to your taxes/work won't change your last name until you've provided proof that you have changed your name with the Social Security Office.  Some make take the letter they provide you as proof but they may require the new card.  I think the banks just require a new license and marriage certificate.

    I used MissnowMrs.com. It was awesome! It costs like $20, but totally worth it in my opinion. The program is interactive. You input all of your info then just print and submit all of the forms--SUPER EASY!!

    Yeah I work at the bank so all we require is the marriage license. And I know I have to send that in for my new SS card too

    If you have a SS office near you, you can do the name change in person.  I did the license and then went and order my new ss card all within about an hour one morning.  Hit the DMV when they opened and then the SS office it was pretty smooth.

    ok this may be a stupid question but I am confused! The certificate we got after the wedding signed by our witnesses is that our offical license? We got a sheet about ordering copies but I didn't know if we had to do that for the offical copy? Help! lol

    No its not the official one, its just one to frame. You have to order the official one to get your name changed everywhere.
