  I think that you should include "Mr. and Mrs." but I would say that whether you include just your dad's name or both is probably up to what you think is best.  I know that some women do not like to be called "Mrs. John Smith".  Since it is not a casual affair, I would definitely recommend that you use the "Mr. and Mrs.".

  Thanks!  That's what I was thinking.  Mom just wanted me to double check.  She thought Mr. and Mrs. with both of their names was too much, but I thought it was correct.

  Coming into the conversation a little late, but I agree that it should be Mr. and Mrs.
  My parents are pretty traditional, so my invitations said
  
  Mr. and Mrs. John A. Smith
  invite you to share in their joy
  at the marriage of their daughter
  FutureMrs

  I would go with Mr. & Mrs. John Smith- but I am traditional about things
