  Are you using outer and inner envelopes?  If so children under 18 do not go on the outer envelope. 
  
  Outer Envelope:
  Mrs. First Last and Mr. First Last
  Address
  Adress
  
  Inner Envelope:
  Mrs. Last Name and Mr. Last Name
  Child First Name and Child First Name
  
  That's how we did it.
