What is the standard way to word the RSVP card? I've seen:

M_____________

__ Accepts with Pleasure

__ Declines with Regrets

and

__ Accepts

__ Regrets

And how do you ask for the number of people who will be attending?

    I prefer the second. I find it presumptuous to attach emotion to accepting or declining. 

    On your second question, just add another blank that says Number Attending? Seems the simplest way to do it.

    Is it better to do:

    __ Regrets

    or just:

    __ Declines

    And what should it say at the top? "The pleasure of your reply is requested by X?"
