  I did it myself in PS. I saw one like it that I loved, so I just tweaked it to make it something I was capable of designing. It's a bit different than the original, but very similar.
  I'm printing through the company I use for my business and they should cost around $50 printed with envelopes. Can't beat that with a stick... LOL
