Marriage Licence/Name Change forms?

Just trying to get a little head here. Which is hard to do planning a wedding. lol

First: Marriage Licence is there a form I can print off and take to the court house? Only thing Im finding on line is requesting a copy of a licence. 

For name change at SS office only online form I came up with is application for a card. Is that the form I want?

    For the license: there are a couple of counties that have an online form, most do not. But when we went to the Chester County courthouse there was no one there so it was NBD. No idea about the SS card.

    We're getting our license in York, and you can fill out the form online to speed up the trip to the courthouse to get the license.  But as pp said, not all counties have this option.

    As for SSN - yep, fill out the replacement/new card form with your new name in the first field -- there's a line right under that for your old name.
