  Maybe just put him down as your step-father? I know he technically isn't yet, but in theory he will be. My daughter already calls my fiancee her step-dad.....you don't need the marriage contract to assign status to people (my fiancee has been my "hubby" for a few years now lol) Has he been in your life for a long time? If so, maybe just listing them as your parents but using the different last names would work, if he has been a father-figure to you. If not, think about the step-father thing. Thats all I can think of for now. Hope this helps!

  Unless he or your mom would be hurt, I don't see him as needing to be included in the program if he's in no way involved with the ceremony.
