Tipping - Help Needed!

Hi Guys!
My wedding is quickly approaching and I'm completely clueless on who / what and how much to tip people.  Listed below are our vendors.  Any help you guys could provide would be greatly appreciated!! 

1.  Florist - Owner is our contact.  I've heard you don't tip the owner?
2.  Photographer - Owner and her husband are our photogs.  Same applies here?
3.  DJ - Owner could possibly be our DJ but not determined yet.  What's a standard DJ tip?
4.  Reception site - Contract includes a 22% service charge but lists it as not a tip.  What have people done in this situation?  We have a catering manager who's been super helpful - she'll also be our Day of Coordinator at the reception site so we'll tip her but any ideas on amount?  What about wait staff?
5.  Limo - Was thinking standard 15%?
6.  Church - We pay a donation but there's an organist and a cantor do people typically tip them?
7.  Hair/Make-up - Independent Contractor - gave her 20% at trial was just going to continue that "trend" although could get super pricey but now I feel like I can't do less because I've set the standard (if she ever gets back to me with a contract that is).
8.  Angie - Does anyone tip her?  My first fitting is next Tuesday and I'm super excited for her to work her magic on my dress!! 

I think that's everyone but if you guys have any advice I would greatly appreciate it!!

    1.  Usually you don't tip florists.  Only the delivery guy and $5-10 is good for him 
    2. My photog is the owner of her company and so we're not tipping her.  (We're having the wedding at a winery so we are giving her a bottle of wine to take home)
    3. DJ same as above
    4. I heard different things about how much to tip wait staff.  I'm also waiting for my final contract from the caterer to see if that has already been added.  I think I'm going to go with $150 for catering manager, $50 per chef, $30 per waiter/bartender.  But I'll be interested to see what other people say.
    7. I'm not sure about my make up person she's an independent contractor, a little pricey and is also charging me $50 to come to my site.  I'm starting to feel that $50 extra is her tip.  I did tip her during the trial, and will probably tip her this time but it wont be 20% more like 10-15%.
    8. I don't think I'm tipping Angie but I am giving her a bottle of wine from our winery.

    All I can help with is:

    Caterer - we're tipping 18 to 20 percent of the total bill (food, rentals, and staff)
    Limo - yeah we'll probably do 15%
    Angie - I tipped her but only $25 on a $225 bill.  I really did it because she fixed my veil but didn't increase her original quote which was just for my dress.

    Sometimes the Limo company will already have a gratuity fee included.  Make sure you check your contract.
