Invitation Wording Help (Response Cards)

We are having family style dinner so there is no entree choice, but as a vegetarian myself I appreciate knowing there will be a vegetarian option. How do I word this on my response cards? I just need to know if anyone is a vegetarian or would prefer a vegetarian entree?

We are just putting a line that says, "Please indicate if you need a vegetarian meal or have any allergies or dietary restrictions"

I put a line on mine asking anyone to contact me if they have dietary restrictions.
