Anyone who applied for an Oakland County Marriage license...

Preferably online...

I think i filled ours out wrong.

For this example, let's just assume my current (unmarried) name is Laura Smith, and when I get married, it will be Laura Doe

The fields were (for the female):
Maiden Name:
Last name:
First Name

How was I supposed to fill this out? 

Option 1
Maiden Name: Smith
Last name: Smith
First Name: Laura

or
Option 2
Maiden Name: Smith
Last name: Doe
First Name: Laura

Thoughts?

    I picked option one.  I think the reason they have a maiden name field would be if this were your second marriage and you didn't change your last name back to your maiden name after you were divorced.  They would want to know what your maiden name was before you were married.  And I looked at the application form online, and "maiden name" isn't a required field.  You could always call (and probably be on hold forever) :P

    yep, definitely calling as soon as I get into work tomorrow. so confused!  Of course they say their busiest office times are between 11:30-1, and I get to work at 8:30 PST (11:30 in Micihgan) and have meetings most of the day  starting at 1, so here's to hoping I won't be on the phone for that entire hour and a half :)

    Yep exactly this.  Basically they want to see that it matches your birth certificate.  Everything must be identical.

    You don't put any information on what your name will be after the wedding on your applicaiton because it's not relevent at this point.  They want to know who you are now, not who you will be.

    Good luck getting in touch with them!  If it helps, I'm sure this happens a lot.

    Thanks Liz, I did fill it out right then (maiden and maiden again) but I definitely had a moment of panic.

    I'm really curious - I saw a marriage license/certiicate (the thing you sign at the wedding) for a friend that got married in Minnesota and it had her maiden name at the top (Rachel Smith), then where she signed, it printed her new last name (Rachel Doe) and she signed there.  Is that how th eMichigan license looks, or would I just saign Laura Doe and be done w/ it?  For some reason I'm super stressing about this license - it's the only part of the wedding that has to go right! :)

    That must be a Minnesota thing.  The MI license doesn't print anything about your new last name and the only way my name is displayed on the license is Elizabeth MaidenName.

    I signed the license in the "signature of bride" section with my new name:  Elizabeth NewLastName.

    Thanks for posting this - I probably would have had the same question. I'm applying online this weekend :) Hopefully I don't mess it up!

    You've got a PM!  Did they tell you when you applied to sign it with your new last name? Is that how you knew?

    You've got mail Laura!  Also, they didn't tell me to sign with my new name but my MOH reminded me.  I'm so thankful she did because otherwise I totally would have forgotten which would have been a nightmare.

    And I wouldn't be to worried about screwing anything up.  They go over it and match everything to the documents you bring with you to check for errors.  My mom's name was listed differently on the birth certificate than what I put on my marriage license application so they tweaked it a bit.

    They will ask you to look over everything and make any changes/corrections.  Once you've ok'd everything, they print off the three copies for you to take with you.  It really wasn't that bad at all.

    Even the security guards are helpful at the county building!  They saw me coming with my wedding binder and asked me if I needed help finding where to pick up the marriage license.  They do this every day and know that people will make some errors.
