  i'm sorry to hear about your mother.  most smokers know to take it outside.
  
  if it's a small wedding, you probably know all of your guests, and can probably ask them discreetly and kindly.

  Is the venue smoke-free?  If it is, I'm sure they'll have signs posted.

  i have a hard time imagining someone blazing up in the middle of a wedding, even if it is an outdoor ceremony.
  
  what is the venue policy?  are they non-smoking? 
  
  this may be something you could include in the program.  something like "due to the health condition of some of our guests, please refrain from smoking."
  
  if you have people that do smoke at the wedding, maybe you can designate a spot for them?   Given that you live in S. Carolina, i would imagine you don't have state smoking laws to fall back on here.

  I'm sorry your Mom has emphesyma. Hopefully you can let people know and they'll be respectful. You might also make sure Mom has an oxygen tank, just in case you've got a few aholes.
