  Yes, all the bridesmaids generally have bouquets. The MOH can have a larger one or not; it doesn't matter. I would advise not, since she'll be holding your bouquet for you as well. If you're looking to save money on flowers, they can carry small bouquets (called a posy or nosegay), a single stem, a pomander ball, or no flowers at all. Or they could have corsages.

  It depends. There is really no right or wrong answer.
  Some brides do a larger bouquet for the MOH. Some do the same size, but maybe a different color. Some do the bouquets all the same. Some dont' do bouquets at all.
  For my friend's wedding, she gave me (the MOH) a beautiful bouquet and her bridesmaids really nice wrist corsages.
  
  I would talk to the florist and come up with a couple of good ideas that you like and play around with it.

  My bridesmaids all had the exact same bouquet, which was a smaller version of mine.

  It was a fairytale.

  Boquets are the norm, but they should be carrying something if they don't have one.
  
  For my mother's evening wedding, her BP carried hurricane lamps with candels.

  It's very common, but there isn't a rule about it.
  
  If you're not carrying anything, just use your hands the way you normally would when you walk.

  Thank you all for your input!  I will definitely get the bridesmaids and MOH small bouquets to hold that are smaller versions of mine.  That sounds easiest.
