Addressing Invites

I've been searching the board for the answers to my questions but I wasn't able to find them, so I apologize if this has been covered.

I'm working preparing the start addressing our wedding invites. 
1) Is this formal enough for addressing married couples - "Mr. & Mrs. Mark LastName" 

2) What is the proper etiquette for addressing family / friends that are over 18 and are dating someone. Is "Mr. LastName & guest" acceptable?  Or should I write "Mr. LastName & Ms. LastName"? Is the reverse of this (for example inviting my sister and her bf) should I address the envelope as "Ms. LastName & Mr. LastName"?

3) We are inviting some family members / friends that we believe are single. But we want them to bring a guest if they would like. In this case is the "Ms. LastName & guest" acceptable?

    According to Crane's:

    1. Yes

    2. Mr. John Smith
        Miss (or Ms.) Jane Doe
    His address

    Reverse the order if the invitation goes to her address. 

    3. Try to find out who they will bring if they know, and address them by name as in # 2. If they don't know yet, "and guest" is fine.
